MILLENNIUM HOME HEALTH is seeking licensed Physical Therapists (PT) and Physical Therapist Assistants (PTA) to join our home health team serving Santa Clarita Valley, Sylmar, and surrounding areas.
We are contracted with a major healthcare network and receive ongoing patient referrals, offering clinicians the flexibility to build their schedule based on availability and caseload.
Job Summary
Provide skilled physical therapy services to patients in their homes, supporting recovery, mobility, and independence. This is a flexible, field-based role working with adult and geriatric patients.
Work Structure & Compensation
- Per diem / per visit position
- Flexible schedule based on patient availability
- Patient volume driven by referral flow
- Opportunity to build a consistent caseload over time
- Full-time equivalent earnings may reach $145,000 – $160,000+ annually depending on volume
Responsibilities
- Conduct patient evaluations and assessments (PT)
- Develop and implement care plans (PT)
- Provide skilled therapy treatments (PT/PTA)
- Educate patients and caregivers
- Collaborate with physicians and care teams
- Complete accurate and timely documentation
Qualifications
- Active California PT or PTA license
- Home health experience preferred (not required)
- Strong communication and documentation skills
- Ability to work independently in the field
- Reliable transportation
